This year's program is points based. Health premium discounts will be earned based on the points level achieved. Individuals can earn a maximum of 500 points. Both coworker and on-plan spouse can each earn a maximum 500 points. The points that the coworker and on-plan spouse each earn will then be combined together to determine the discount level. Completion of the Biometric Screening and Marathon Health Questionnaire (MHQ) are required in order to earn any other points in the wellness program. You can start entering points for the self-reported items at any time. Once the Biometric Screening and MHQ are complete, the additional activity points will be added to your total points. BOTH coworker and on-plan spouse must complete BOTH the Biometric Screening and MHQ by 9/30/2021 in order to earn points.
